    1. Name of the Parking Place and parking lots required
    2. Period of use (Date / Time)
    3. The address of intended use (e.g. address of the unit under renovation/construction, address for loading/unloading goods)
    4. Your Company Registration No. (UEN) if you are applying on behalf of your company
    5. Other relevant supporting documents for Religious/Festive Activities (a copy of approval letter from MP, LTA, TP etc) if applicable.
